Consultant Insurance Requirements

TxDOT requires every provider to maintain the correct insurance coverage depending upon the project.

TxDOT Form 1560-CSS is a certificate of insurance form relevant for providers selected to perform the professional services listed. Submittal information is listed at the bottom of the form.

Current proof of insurance must be on file with TxDOT's General Services Division, Contract Services Section prior to contract execution.
