What should I do if I cannot open or view an online form? |

Many of our online forms
are in a PDF format that can only be viewed or printed with
Adobe Acrobat Reader 7.0 or higher. If you don't have Adobe
Acrobat Reader or are using an older version, you won't be
able to open the forms. Install or upgrade to a newer
version; you can
download it free. It is important that you first
uninstall any prior versions of the Reader before
downloading and installing the latest version.
You also could be experiencing problems for other reasons:
- Your Internet Service Provider (ISP) might be
experiencing heavy traffic.
- The file size could be too large for your Internet
connection speed. Read the FAQ on how to download a form
directly to your computer's hard drive.

What should I do if the
form looks like it has loaded but does not open? |

If this happens, refresh your Web browser. If that fails, make sure you have
Adobe Acrobat Reader 7.0 or higher installed properly. If you do not have Adobe
Acrobat Reader, you can
download it free.

How can I bypass opening
the form in the Web browser and download it directly to my desktop or
hard drive? |

PC - Microsoft Windows
Use the "Save Target As..." option to download the PDF file directly to your
hard drive:
- Go to the page containing the link of the form you are trying to save.
- Move your mouse pointer over the link on the page.
- Right click (or left click if your mouse is configured for left-handed
operation) on the link.
- Click on "Save Target As..." when presented with a menu.
- Select a location on your PC to save the file.
Apple Mac OS
Use the "Save Linked File to 'Downloads'" option to download the PDF file directly to your hard drive:
- Go to the page containing the link of the form you are trying to save.
- Move your mouse pointer over the link on the page.
- Right click on the link
- Click "Save Linked File to 'Downloads'" when presented with a menu.
- Click or launch Finder > open Downloads folder. File is saved as "GetForm.pdf."
Use Adobe Acrobat Reader to open the file after downloading. If you do not have
Adobe Acrobat Reader, you can
download it free.

When downloading a PDF
form, my Web browser window says, "Done," but all I get is a blank
screen. I already have the latest version of Adobe Acrobat Reader. What
should I do? |

If a PDF file appears as a blank window within your web browser after
downloading, follow these steps to resolve the issue:
- Click your browser's "Refresh" button.
- Verify that you have the correct version of Adobe Acrobat Reader. If you are
using a version prior to 7.0, upgrade to version 7.0 or later. You can
download it free.
- Try using the "Save Target As..." (or "Save Link As...") option to download
the PDF file directly to your hard drive and bypass the Acrobat Reader plug-in.

When I download a PDF form,
I get a message that the PDF file is damaged or corrupted. Is your file
damaged? |

All of our PDF files are verified prior to posting on the Web site. It's likely
your download was not successful. Exit your browser and try to download the file
again. If you are still experiencing problems, please try possible solutions
addressed by Questions 1-4 before calling forms system support at (866)
560-4755.

When I tried to download a
PDF form, I got a message asking me what application to use. I selected
Word (or another program). Now all I see are weird symbols when I look
at the file. What can I do? |

The easiest way to fix your problem, is to
install Adobe Acrobat Reader. Once the installation is complete, your
computer will no longer use Word to view the PDF file. Your computer will use
Adobe Acrobat Reader to view the files instead.

When printing some forms in
PDF format all the words run together with no spaces between them;
however, everything looks fine on the screen. What is causing this? |

There are a variety of reasons that could cause a PDF file not to print
properly. Please see Adobe's Support Knowledgebase on
troubleshooting printing problems.This issue can often be resolved by
obtaining and installing the most recent print driver available for your
particular make and model printer. Most printer manufacturers provide free
printer driver updates from their Web sites.

How do I find out which
version of Adobe Acrobat Reader I use? |

Locate the Acrobat Reader program on your machine and open
the program (you probably have an icon on your desktop for
Acrobat Reader that you can double-click). Click on the
"Help" menu item at the top of the Acrobat Reader window.
Select "About Acrobat Reader" from the menu options. A
window will appear that displays the version.

How do I open a form using an Apple computer? |

Firefox Procedures
- Go to the web page for the PDF form you want to open.
- Click the mini-Adobe PDF icon
.
- You should get a small pop-up that asks what you want Firefox to do.
- Select “Open with”.
- Change the “Preview (default)” setting to “Adobe Reader.app.”
- Select “OK”.
- The form will download to your “Downloads” folder by default.
- Adobe Reader will automatically open the form.
- A pop-up will appear asking, “Would you like to make PDF files open
with Adobe Reader instead of Preview?”
- Select “Yes.”
From now on, Adobe Reader will open PDF files instead of Preview.
Safari Procedures
- Go to the web page for the PDF form you want to open.
- Click the mini-Adobe PDF icon
.
- Download the PDF to your desktop (or prefered location).
- A pop-up window will appear that states, “To view the full contents of this
document, you need a later version of the PDF viewer…,”.
- Hover your mouse over the bottom-middle of the page. Four options will
appear:
- Zoom Out
- Zoom In
- Open PDF in Preview
- Save PDF to Download Folder
- Select, “Save PDF to Download Folder”.
- Open the Finder application.
- Select the “Downloads” folder.
- Right-click “GetForm.pdf”.
- Highlight “Open With”.
- Select “Adobe Reader.”
- Adobe Reader will open the form automatically. A pop-up message will
appear asking, “Would you like to make PDF files open with Adobe Reader
instead of Preview?”
- Select "Yes".
From now on, Adobe Reader will open PDF files instead of Preview.
