Form VTR-356 (Schedule A and Schedule B) can be viewed and printed using Adobe Reader.  The default form dimensions are 14" X 8.5", however, it can be printed on 8.5" X 11" paper. 

Once you have clicked on the link below, follow these steps to print the forms on 8.5" X 11" paper:

• click on File (in the menu bar) and then click Print
  or click on the Printer Icon at the top of the screen.

• Once the print dialog box has appeared, click on the "Shrink to Fit"  box (this forces the forms to be printed on an 8.5" X 11" page.

• Click the "OK" button.

For Word Format

1. Click on the link labeled "in Word" to download the file. You will be given the option to save the document to the directory (folder) of your choice. Once you have chosen a directory the file will be downloaded to your computer with an .EXE extension.
2. Open Windows Explorer (File Manager) and double click on the downloaded .EXE file. Choose a folder to unzip the file.  The .EXE file will be unzipped and appear as a .DOT file in the folder you chose.
3. You can then open the .DOT file through Windows Explorer or MS Word.
