From SH 195, 1.0 mile north of Florence, westward approximately 6.3 miles to US 183. (Williamson County)  As requested by District, 6.3 miles of SH 195 redesignated and transferred to SH 138.  New Designation.

North of the city of Florence, SH 138 was extended along a new location on the state highway system from the former alignment of SH 195 (redesignated as BS 195-H) eastward to the new alignment of SH 195, a distance of approximately 0.6 mile. (Williamson County) Extension: On New Location
